Operative Concept
Revision surgery under endoscopic visualization
Revision endoscopy emphasizes altered anatomy, scar tissue, and a narrowly defined operative target.
Altered anatomy
Prior decompression or fusion changes landmarks, removes bone, and may create scar tissue around neural structures. Preoperative imaging and prior operative details are central.
Targeted corridor
The endoscopic corridor should be selected to reach a specific recurrent compression while reducing unnecessary dissection through scarred tissue.
Decision threshold
Revision surgery should proceed only when the expected surgical endpoint is specific enough to justify the increased risk environment.
